2 Days Less Sleep. Ghrelin +28%. Hunger +24%. Your Body Is Sabotaging Your Diet.

Two days of reduced sleep increase ghrelin by 28% and hunger by 24%. The landmark randomised crossover study by Spiegel, Tasali and Van Cauter.

Updated October 2, 2026

Annals of Internal Medicine (2004), randomised crossover study by Spiegel, Tasali and Van Cauter: sleep restriction of just 2 days induces an average 18% reduction in leptin and 28% increase in ghrelin, with a 24% increase in self-reported hunger.

The experimental design: controlled conditions

Participants were under controlled conditions of caloric intake and physical activity.

The hormonal variations are direct, causal effects of sleep restriction — independent of diet or exercise changes.

The biology of sleep-deprivation hunger

Ghrelin is the only known orexigenic hormone. Leptin is the main anorexigenic hormone.

When ghrelin rises 28% and leptin falls 18% simultaneously, the brain receives a double signal: 'I am hungry' and 'I am not full'. Following a diet under these conditions is biologically extremely difficult.

The link between sleep and diet failure

With just 2 nights of reduced sleep (even 7 to 6 hours) hormonal control of appetite deteriorates. Those who chronically sleep little are in a state of hormonally amplified appetite — and any diet fails without correcting this imbalance.

The Digital Twin as indirect hormonal control

The Digital Twin monitors sleep quality — the causal driver of hormonal alteration — and generates alerts when the sleep profile enters the risk zone for appetite control.
